Case Manager for Service Members, Veterans, and Families

This position is responsible for managing an assigned caseload of Identified Veteran status clients, both active and inactive, and having a major mental disorder as diagnosed using the Diagnostic and Statistical Manual of Mental Disorders. These disorders are considered severe, recurrent, and long-term such as schizophrenia, major affective disorders, paranoia, organic or other psychotic disorders, personality disorders, or disorders that may lead to chronic disability. This position is responsible for assisting these individuals, who reside in a community setting, in gaining access to needed medical, social, educational, and other services. This position is responsible for conducting assessments to determine appropriateness for Services.

QUALIFICATIONS:

  • A combination of mental health work experience or relevant education, and training, which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties required.
  • Bachelor’s degree earned from an accredited college or university in a relevant human services field required.
  • At least one-year experience working with adults
  • Qualified Mental Health Professional (QMHP) eligibility according to DMAS requirements required.
  • Must possess a valid Driver’s License, good driving record and the ability to provide your own transportation.

ADDITIONAL REQUIREMENTS:

  • May be exposed to allergenic environments from client’s homes, including extreme heat, humidity, dust, smoke, pet contamination, cleaning supplies, etc.
  • May be exposed to clients with infectious diseases
  • Must be able to drive for long periods of time to visit clients in and out of the catchment area as needed
  • Must be able to walk distances, climb and descend stairs, maneuver in and out of agency vehicles
  • Prolonged periods sitting at a desk and working on a computer
  • Must be able to lift/push/pull up to 25 pounds at times

SALARY: Starting at $52,788

RETIREMENT:  NWCSB participates in Virginia Retirement System.
