This position is responsible for providing training and consultation for new case management staff assigned as case managers. This position is also responsible for managing an assigned caseload of individuals seeking mental health, behavioral health, and/or substance use disorder services. The case manager is responsible for assessing needs, developing, implementing and reviewing service plans, and working with other community resources to meet client service needs and provide quality care.

QUALIFICATIONS:
- A combination of mental health work experience or relevant education, and training, which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties required.
- Bachelor’s degree earned from an accredited college or university in a relevant human services field required.
- At least one year of experience working with children required.
- Qualified Mental Health Professional (QMHP) required.
- Must possess a valid driver’s license, a good driving record, and the ability to provide your own transportation.
ADDITIONAL REQUIREMENTS:
- May be exposed to allergenic environments from client’s homes, including dust, smoke, pet contamination, cleaning supplies, etc
- May be exposed to clients with infectious diseases.
- Must be able to drive for long periods of time to travel in and out of the catchment area as needed.
- Must be able to walk distances, climb and descend stairs, maneuver in and out of agency vehicles.
- Prolonged periods sitting at a desk and working on a computer; and
- Must be able to lift/push/pull up to 25 pounds at times
SALARY: Starting at $56,549
