This position requires the incumbent to conduct initial assessments including, but not limited to, the Adult and/or New Client Assessment. The incumbent will determine appropriateness for Targeted Case Management Services, Crisis Intervention Appointments, and Health Planning Region (HPR) I Detox Screening. The incumbent will meet with a wide variety of individuals seeking psychiatric services at Northwestern Community Services Board (NWCSB) as part of the intake process. Case management and clinical interview skills are needed to obtain previous client records from outside agencies and collect relevant historical information from the individual. The SDA Service Intake Therapist will also provide coverage for individual therapy, as directed.

MAJOR DUTIES AND RESPONSIBILITIES:
- Provides initial intake sessions for new clients via the SDA Walk-in Clinic, providing short-term crisis intervention counseling for new clients, when needed.
- Gathers psychosocial and treatment history on new clients. Provides referrals for community resources, as needed.
- Completes case management eligibility and co-occurring assessments for clients being accepted into services.
- Conducts SDA follow up appointments to assist in monitoring of clients and maintaining initial treatment engagement call while awaiting psychiatric appointment, or initiation of substance use treatment services or substance use residential treatment placement.
- Coordinates referrals of clients with supervisory staff to facilitate continuity of care.
- Submits all documentation in a timely manner; ensures releases of information are signed; and requests appropriate historical treatment.
- Conducts detox/rehab assessments as directed.
- Provides support, as needed, for the exploration and development of new programs.
- Completes Intake Assessments for all service lines and coordinates follow-up care, as needed.
- Conducts individual therapy, as directed.
- Performs other duties as assigned by the supervisor.
QUALIFICATIONS:
- A combination of mental health work experience or relevant education, and training, which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties required.
- Master’s degree earned from an accredited college or university in relevant human services field required.
- LMHP eligibility required, license preferred.
- Experience or relevant course work with completing assessments and providing basic counseling interventions required.
- Must possess a valid Driver’s License, good driving record, and the ability to provide your own transportation.
ADDITIONAL REQUIREMENTS:
- May be exposed to allergenic environments from client’s homes, including dust, smoke, pet contamination, cleaning supplies, etc.
- May be exposed to clients with infectious diseases
- Must be able to drive for long periods of time to travel in and out of the catchment area as needed
- Must be able to walk distances, climb and descend stairs, maneuver in and out of agency vehicles
- Prolonged periods sitting at a desk and working on a computer
- Must be able to lift/push/pull up to 25 pounds at times
- Must possess a valid Driver’s License, good driving record and the ability to provide own transportation
SALARY: $61,789 – $78,611
